Case study: Serious case review and systems analysis for a London Borough

Client needs: PSI was employed to provide an objective review of the processes used by the local authority in managing a serious incident involving a school aged child and their family

What we did: The process of the review was based on current national guidance and the recommendations of the Laming report. A soft systems analysis was used to identify the range and activity of all of the relevant agencies and parties involved. Evidence was collated to provide a detailed chronology and report. Confirmation of process, accuracy of the evidence and decision-making by those involved took place in multiagency meetings during the period of the review.

Outcomes: Following the detailed review recommendations were made for consideration of changes in a number of areas important to the effective management of complex case situations:-

  • internal procedures;
  • case management ;
  • reduction of risk of future incidents;
  • interagency liaison.

The independent review carried out by personnel familiar with the legislation and local authority procedures associated with safeguarding and special educational needs enabled the authority to review and refine the processes in place to manage complex cases.
